Solon Retirement Village is looking for a Care Plan Coordinator to join our nursing management team!

Solon Retirement Village is a community owned campus including independent living, assisted living, memory care, skilled nursing and long term care who has been serving our community for over 45 years!

Primary Duties include:

ICF assessments and care plans for our 92 bed long term care facility.

May be on call in our management rotation.

Must be an RN, experience in long term care preferred.
