Come and check out our new long term care unit, The ARBOR AREA! The Arbor Area is a transitional unit that focuses on maintaining independence. Consisting of 12 private rooms, this intimate setting offers its own dining and living area as well as a spacious outdoor patio.
Our full-time social worker assisting with:
- Families and hospital discharge planners from initial inquiry.
- Serves as an advocate and provides counseling services as part of the Care Plan Team.
- Coordinates discharge planning following completion of therapy program, to return home or to a lesser level of care including home health services.
- Provides guidance in navigating the Medicaid application process.
